# Uploading Documents to Patient Charts

Upload PDFs to a linked patient (25MB max, 0.25 credits). How to attach them during recording or note writing.

**You must link a patient to your session before uploading documents.** Uploading without a linked patient means documents won’t appear or be accessible.

## Upload options

**From the patient chart:** Navigate to the patient’s chart and go to the Documents section.

**During a recording session:** Link your patient first, then upload documents using the Recorder. Documents must be uploaded before starting the recording.

## Using previously uploaded documents

When writing a note, uploading audio, or starting a new recording session, you can select documents already in the patient’s chart. The patient must be linked to access these files.

Useful uploads: lab work, intake forms, prior records, imaging reports, specialist consultations.

## File requirements and credit usage

 - PDF files only, maximum 25MB
 - Each uploaded document uses 0.25 additional note credits
 - Documents over 10 pages use an additional 0.25 credits per extra page
 - Credit terms may vary if your clinic has a specific agreement with us
