#### Abstract

Across 312 buprenorphine inductions over 8 quarters, retention at 90 days was 78%, with COWS reduction of 9.4 (95% CI 8.7 to 10.1) from baseline. Concurrent counseling participation averaged 71% (n=222 of 312). 14 inductions were terminated for diversion concerns; 7 of 14 transitioned to a higher level of care.

#### Methods

Retrospective analysis of all signed encounters tagged MAT in the workspace. COWS scores, UDS results, and counseling attendance pulled from the structured fields on each visit note. No free-text mining; results derive from the same fields the chart uses.

#### Results

 - **Retention 90 days:** 78% (243 of 312)
 - **COWS reduction baseline to wk 12:** 9.4 (CI 8.7 to 10.1)
 - **UDS positive for prescribed buprenorphine at wk 12:** 92%
 - **Counseling adherence:** 71% mean across cohort
 - **Adverse events:** 4 documented, 0 serious

Every claim links to the source encounters. Counts come from structured fields on signed notes. Enterprise adds longer cohort windows, multi-workspace pooling, and custom CI methods.
